Important information for incoming Freshmen
Physicals are required for all incoming freshman on a State of Illinois Health Examination Form. Sports physicals do not count as a freshman physical.

Freshmen are required by state law to provide proof of one dose of the Tdap vaccine, 2 MMR's, the series of 3 Hepatitis B vaccines, 3 doses of the Polio vaccine, and 2 doses of the Varicella vaccine or proof of the chickenpox disease to be enrolled in school.

It is the responsibility of the parent or guardian to provide the current immunization record with the physical exam at the time of registration

Medication Permission Form Please Fill Out - This Parents and Guardians of all new incoming freshman's parents must also fill out the Medication Permission Form.
Important Information for Students and Parents
All physicals & immunizations must be completed by October 1st or the student will be considered non-compliant & excluded from school.

All students entering 12th grade MUST show proof of 2 doses of the MCV4 (meningitis) at school entry. The 2nd dose must be administered on or after the 16th birthday. If the 1st dose is received at 16 years of age or older, only 1 dose is required.
