Important information for Students

LCHS requires a current physical exam of every entering freshman and all transfer students.

The State of Illinois requires all students to provide proof of: • One dose of the Tdap vaccine • 2 MMR’s • The series of 3 Hepatitis B vaccines • 2 doses of Varicella or proof of the chickenpox disease

All incoming seniors will need proof of two meningitis vaccine, the second given after age 16.

It is the responsibility of the parent or guardian to provide the current immunization record with physical exam form at the time of registration. You will also need to show proof of a current school dental exam.

All physicals and immunizations records must be completed by the first day of school. If a student does not comply they will be excluded from school after that date. The student will be considered truant because of noncompliance. For transfer students, the physicals will be required within 30 days of entering LCHS.

Sports physicals for 10th, 11th, and 12th grade are required before the athlete can try-out. Physicals are good for ONE year from the date of the exam.

ALL medication must be kept in the nurse’s office and dispensed by the school nurse. A signed physician’s order is required for all medications administered at school. This includes all over–the-counter medication.

Important Information for Students and Parents

All physicals & immunizations must be completed by October 1st or the student will be considered non-compliant & excluded from school.

All students entering 12th grade MUST show proof of 2 doses of the MCV4 (meningitis) at school entry. The 2nd dose must be administered on or after the 16th birthday. If the 1st dose is received at 16 years of age or older, only 1 dose is required.
